Dear Parent/Guardian:
According to the Education Law and Regulations of the Commissioner of Education it is required to have a physical examination of children when:
1. Enter the school the school district for the first time.
2. Are in grade K,2,4,7,10.
3. Participate in interscholastic sports.
4. Need working papers
5. Are referred by/to the Committee on Special Education.
6. Are deemed necessary by school authorities to determine a child’s education program.
The school’s physician can do this physical, however, the child’s primary health care provider may also perform the health appraisal. If you choose to have your child’s primary care giver to perform the examination please contact the health office so that I may send you a copy of the necessary form needed. If I do not hear from you I will assume that you would like to have our District’s physician complete the physical. The examination will be done be Dr. Lori Hudzinski or one of her associates.
The physical examination should include the following:
General appearance
Hair and scalp
Skin
Lymph glands
Eyes, Ears, Nose, Mouth
Neck
Cardiovascular
Chest, Lungs
Abdomen
Genitalia (for the male an examination of the presence or absence of hernias)
Musculoskeletal
Neurologic
Height, weight, vision and hearing are performed by the school personnel
